It sounds like you have a Fulton or Cequent branded 1,000 capacity jack and are in need of a repair kit. The part # F0933302S00 is a repair kit for Fulton jacks with a weight capacity of 800-1,200 lbs.

Based on the picture you submitted this should be what you would need.
