The Kodiak Disc Brake Kit - 10" Hub/Rotor - 5 on 4-1/2 item # K2HR35D is a great disc brake conversion kit. It is rated for 3,500 lbs axles, has a 5 on 4 1/2" lug pattern, and is rated for use on 13" wheels. This disc brake kit fits axles with diameters of 1.06 inch, 1.38 inch, and 1.73 inch. You will want to use a digital caliper like the item # PTW80157 to measure where the seal and bearings ride on the spindle. I have linked a diagram that demonstrates what measurements you will need.
The HydraStar Electric Over Hydraulic Actuator Kit for Disc Brakes - 1,600 psi item # HBA16-252-82 is the way to go to get everything you need. It includes the actuator, breakaway kit, lines, and fittings.
