I reached out to my contact at Dexter Axle who said that based off of the numbers you sent in it sounds like you have a #11 Torsion axle rated for 5,200 lbs that is on a Haulmark Trailer manufactured back in 2001 (at least the axle was). Your axle should use 6 lug hubs with EZ-Lube spindles and that it has a 95" hub-face measurement with a 75-3/4" outside bracket measurement (which would be equivalent to spring-center).
The closest thing that we have to this is the Dexter Trailer Axle w/ Electric Brakes # 6340624-EB which has EZ-Lube spindles, a 6 on 5-1/2" bolt pattern, is rated for 5,200 lbs, and has a hub-face measurement of 95". The only problems with this is that the spring-center is 80" and you would need to switch from Torsion axle to standard suspension components which requires you to relocate the spring seats and purchase/install all new suspension. Attached is a link to our suspension components but if you want to go this route I can help you choose the correct parts.
The alternative to this is using a Timbren Axle-Less Trailer Suspension System. These come in the following configurations rated for 5,200 lbs:
4" Drop - # ASR5200S04
Standard - # ASR5200S01
2" Lift - # TASR5200S05
4" Lift - # TASR5200S03
If you like how the Torsion axles work then I highly recommend going with one of these options instead. They operate in the same manner and help to retain ground clearance by not requiring an axle beam (although some applications may require a steel crossmember). Since these incorporate the suspension all you would need to complete the setup is the Electric Trailer Brake Kit # AKEBRK-7-SA for each axle and then 4 of the Hub and Drum Assembly # AKHD-655-6-EZ-K.
