Hey Tim, thanks for reaching out!
The Curt Multi-Fit Ball Kit w/ 4" Offset # C68ZV (60663) will only fit your 2021 GMC Sierra 3500 if it has the 8' bed and OEM puck system.
Since you have a short bed, the only offset gooseneck hitch ball kit that will work is the Gen-Y # GY55XR. This actually offers a 5" offset as opposed to the 4" offset of the Curt kit.
The puck pattern for the long and short GM trucks is different in relation to the gooseneck hitch ball hole. Why GM decided to go that route is beyond me, but that's why the Curt kit isn't interchangeable.
