Hey Loren, the best disc brake actuator kit for a triple axle trailer is the Hydrastar kit part # HBA16-252-82 which comes with the best and most reliable actuator from our experience. This will also require an additional brake line kit since it's a tripe axle which we have with the part # 9579-01. Aside from that all of the electrical and hydraulic parts are covered.
Then for the hub/rotors the Kodiak kit part # KOD69FR would be your best option. This would be correct for your axle capacity and bolt pattern. The studs are 1/2 inch in diameter which is most likely what you have but you might check.
