Do I just need replacement side rails for this to fit an oem ford luck system or would I need a rail adapter?

You don't need a rail adapter. To attach the Demco Autoslide 5th Wheel Trailer Hitch w/ Slider item # DM8550035 to your 2023 Ford F-350's puck system you will need the Underbed Mount Adapter Plates item # DM6033.
I have attached a picture below so you can see the difference between a standard Hijacker and one that uses the Adapter Plates item # DM6033. These legs will drop right down into the bed of your F-350.
