Looking for a Trailer Hitch to mount or weld onto our 2006 Chevy Cutaway 3500 Box Truck. the measurement is 42 1/4inch from outside to outside on the back. Wondering if anything here would work or best options if we need to weld something to the back frame. Thank you!

You will need to weld to the frame one way or another to have a hitch installed on your box truck. The hitch on the picture you attached is still connected to the frame underneath, it just runs up through that step bumper. If you wanted to attempt to replicate that setup you could take the Curt Combo Bar # C49360 to a custom fabricator and have them design it to fit your truck.
The other option is to use a universal hitch like the Curt Class V Universal Weld-on Receiver Hitch # 15902. This fits frames up to 44" and has a 2" receiver as well. It has brackets for a 10.865" drop but they can be modified as needed by the certified welder when installed.
