What is the purpose of the third hole that is between and lower than the two primary mounting holes? Is this for a bolt to secure safety chains?

The third hole on the Demco Hydraulic Brake Actuator w/ Electric Lockout - Disc - Zinc - 2" Ball - 7,000 lbs # DM62VR is for an additional bolt for mounting. The other two holes are required for mounting the actuator, the one you are referencing is optional. You could choose to drill a third hole to make use of this or just use the top two.
Safety chains are typically mounted to the trailer. If you need to add mounting brackets for safety chains you would need something like the # MB12 or # MB58.
