According to Blue Ox, users of the Blue Ox # BX8870 KarGard typically don't need an extender to increase the distance between the motorhome and the towed vehicle because the KarGard will sit pretty close to the vehicle being towed (about 1 foot).
If you should find you need an extender, Blue Ox tells me that it's acceptable to use the # BX88264 for 12 inches of extension or # BX88265 for 6 inches of extension, but the tow bar will need to be within 3 inches of level. They go on to say it's NOT acceptable to combine an extension with a high/low adapter like # BX88128. A high/low adapter will already provide some extension.
