Would this be acceptable to use with a Ram 3500 SB with a Reese gooseneck adapter on my fifth wheel.

I do have a recommendation for you as the B&W Extend-A-Goose Gooseneck Coupler # BWTEXA4200 will not work with a Reese Goose Box # RP85FR.

You will need to use a offset gooseneck ball like the 4" Offset Ball # BWGNXA4085 with your gooseneck hitch on your 2019 Ram 3500 short bed. This particular ball is designed for a B&W Turnoverball Underbed Gooseneck Trailer Hitch # BWGNRK1320. If you have the OEM factory hitch prep/puck system you would have to replace the center section of your hitch with the B&W Turnoverball as you cannot use an offset ball with the OEM hitch since it would be able to turn inside the coupler.
