Will this work for my situation, quad in back of truck with tailgate down for quad, pulling trailer.

I do have an option for you but the TorkLift SuperTruss Hitch Extension isn't an option for a GMC Sierra/Chevy Silverado 1500 since they don't offer a trailer hitch for this vehicle needed to use this extension. The longest extension we offer is 18", the Curt Extender for 2" Receivers part # C45796. If you're using a weight distribution system we offer the Equal-i-zer Weight Dist Shank part # EQ90-02-4600.
I couldn't say for sure whether either option will allow you to tow with the tailgate down, you'd need to measure as there are too many variables.
