I need mounting bracket and corresponding hardware for 12 roller along with a 5/8 shaft.

We have the Yates # YR12243-5 side guide roller measuring 12" long with a 5/8" shaft. We also have the CE Smith # CE10305G Panel Bracket for 12" Boat Trailer Rollers in Galvanized Steel.
For roller shafts we have the closest we have is CE Smith # CE10727A measuring 13" X 5/8" shaft, in zinc plated steel.
