Which one comes with the gussets to weld in behind the adjustable hitch, or do I have to make them myself

The Demco Trailer Coupler w/ 5-Position Adjustable Channel part # DM612466-81 is designed to be welded directly to the trailer as you know. With the different type of applications this is intended to fit they do not provide gussets or other reinforcements.

You will want to create you own that will match up with your trailer frame thickness and the height of the coupler.
