I bent/ jackknifed my coupler cover moving to TX, I know it is Titan but can not find the model number...picture attached.

Based on the pictures the correct replacement cover for you is the part # T4324600 which is designed for surge brake actuators rated for 7,000 lbs and use a multi-ball coupler style just like what you have.
Hopefully when you jackknifed your trailer the cover was the only thing that was damaged. It would definitely be a good idea to remove the cover and take a look at the actuator itself to make sure it's still in good shape, otherwise it could be pretty dangerous to tow with that coupler.
If it does look like the integrity of your coupler/actuator has been compromised then we do have replacement options for you. A direct replacement is part # T43397007K.
