Hi, Can it works upside down? so it will have a of 3-1/4 I will town my car with my RV, I imagine that the tow bar should be completely Horizontal. What is the maximal acceptable angle? My car is a Fiat 2013 500 C Thanks

The Curt Dual Hitch Extender # D-192 that you referenced can be flipped if you were to use the bottom receiver for a hitch-mounted accessory (bike rack, cargo carrier, etc), but I don't recommend doing this if you still plan on using the lower receiver for your tow bar. It's just not going to provide a stable/sturdy enough connection when used in this manner.
A better option is going to be with the Roadmaster Dual Hitch Receiver Adapter # RM-077-4, which features a 4" drop and is designed specifically for flat towing. When hooking up to your Fiat the tow bar just needs to be within 3 inches above or below level. This is what is referred to as the "Safety Zone". As long as you are able to get the tow bar within this range you're good to go.
