I have a solution for you, but since you're switching from incandescent to LED lights and taking into consideration the size of light you're going to need, I won't be able to recommend a 4 function light that covers all of those criteria; we also don't carry actual vehicle specific replacement lights, only the bulbs. When all is said and done, because of the issues that can occur when switching from incandescent to LED lights, you're likely going to require some wiring work.
With that being said, I recommend going with the Fusion LED Trailer Tail Lights # STL68RB . These are 4 function lights that will give you Turn Signals, Brake Lights, Backup and Tail light functions in a 10" x 3" LED light. You can add # A68SSB to give you a nice, finished stainless steel trim look as well. Since you're converting from incandescent lights to LED, you may experience some hyper flashing or voltage bleeding. If that occurs, you'll need to install a load resistor similar to # DI34ZR so that your lights function properly.
