Yes, you will need an aftermarket trailer brake controller to equip your Audi Q7 for towing a trailer with brakes. The actual components included in an OEM "tow package" will vary by brand and model but the trailer brake controller itself is not included among them.
If your Q7 has a factory 7-way connector similar to the one shown in the linked photo you have the option for an exceptionally easy brake controller installation.
Curt's Echo Trailer Brake Controller # C51180 requires NO wiring, cutting, splicing or any other parts for installation, apart from your existing smartphone which serves as the display and control device. Plug the Echo into your 7-way, plug the trailer into the Echo, pair it to your smartphone and you're DONE! If you don't have a 7-way you'll add # ETBC7L.
