Will the rack hook adjust to fit a girls 18 inch and a girls 20 inch bikes without having to purchase an adapter?

The Swagman Quad Platform Bike Rack # S64FR referenced in your question will work well with bikes that have as small a wheelbase as 15 inches (or as large as 51 inches) because of its sliding wheel trays that can be adjusted along the base. With this in mind, this rack will work great for both your 18 inch and 20 inch bikes.
The only adapter piece you may need is a bike frame adapter like the Kuat Ubar # UB01 if your bikes feature a pronounced step-through or other alternative-style frame that the padded hooks on the rack might have trouble securing to. This bar will provide a horizontal attachment point for level transport. I have attached a short video demonstration on this product so you can see exactly how it works and why it might be needed.
