Good day, I was wondering if its possible to install it with the spare tire and use only the last 3 positions. Thank you!

The instructions for the Swagman 4 Bike Carrier # S80600 only state to make sure that the weight of the bikes is evenly distributed over the support members.
As long as you don't have heavy bikes on the end you should be good to just use the last 3 bike positions. Just be sure to put your heavier bike in the position closest to your bumper.
If you would like to add a theft-deterrent to this bike rack then I recommend using the cable lock # e98891 as well.
