In order to find the correct replacement hub assembly for your trailer, there are a few things you will need to determine.
You will first want to locate the capacity of your axles, followed by measuring the bolt pattern on your existing hubs. I have attached a great article that explains exactly how to do this. Then, you will need to locate the inner and outer bearing numbers on your hub. This information can be found by pulling the hub and checking for the numbers that should be stamped onto the bearings themselves. If you cannot read the numbers, you can also take a few measurements along the spindle where the bearings sit. I have attached an image that shows where these measurements are taken. I recommend using a digital caliper like # PTW80157, as the measurements will need to be exact.
For example, the etrailer.com Trailer Hub and Drum Assembly # AKHD-545-35-EZ-K is designed for 3,500 pound axles and features a 5 on 4-1/2 inch bolt pattern. It uses the # L68149 inner bearing and # L44649 outer bearing.
