Tire size is 225/65-R17 and rated M+S Do I need snow chains/ cables? And if I do, which ones? Which set of wheels do they go on?

It is always wise to check a vehicle owners manual for any guidance or restrictions regarding use of tire chains. Some versions of a vehicle may be able to use chains while another version might not be able to use them; other cars may be able to use chains with only one specific tire size installed on the vehicle. I reviewed an online edition of the manual for your 2016 RAV4 Hybrid and it indicates low-profile S-Class chains can be installed on the front tires only. Check your printed owner's manual in case it differs from the online version.
