Does this end cap come with the screw? thanks!

The # 8523166001 replacement end cap you referenced doesn't use a screw to fasten, it uses a Thule lock core. This end cap is meant to be used on the bottom of the uprights, as shown in the diagram I've provided.

I believe you're actually looking for part # 852-3214001, which is the end cap for the ladder rack crossbar. The screw that retains the cap in place, is a M6-1 x 14mm Socket Head Cap Screw that isn't included with the replacement cap, and isn't available separately. It is available as part of the # TH752-1391-001 replacement fastener kit.
