Question:

In looking at the instructions I see that some nuts may be already welded on the frame. How do you put the carrriage bolts in with the nuts welded where the carriage bolt heads go? Also, where do the square spacers go? Under the head of the carriage bolt or under the bottom of the frame and above the tow hooks? One last question. How does the fishwire tool work?

Regarding your first question, the carriage bolts will point up and thread from through the tow hooks (if present), and up into the factory weld nuts if they are present as well.

If your vehicle doesn't have the weld nuts then the carriage bolts will instead fish wire through the frame and point down instead. The square spacers will end up in the vehicle frame with the bolts. Basically the bolts will go through the square spacer, through the frame, through the hitch mounting flange, through the tow hooks and then secured with the included nut.

Fishwiring is a technique that involves threading a wire with a bolt and spacer attached to it through the mounting hole and out a portion of the frame so that you can then pull the wire the opposite direction and everything will fit in place. It's a hard process to visualize, but if you check out the video I attached to the right you will see exactly how it works.
