Hi Raymond, yes, you can still open your rear doors with the Surco Rear Door Spare Tire Carrier, part # SU29AR, installed. The carrier is designed to mount to the rear door hinges, so it moves with the door as you open and close it. It also uses an offset design that keeps access to the factory door handle and latch, so you can use your rear doors like you normally would without needing a separate swing away carrier.
It's a nice solution if you're running a larger spare tire or just want to free up cargo space inside the van. Keeping the spare mounted outside means it's easy to access if you ever need it, while still allowing you to load and unload through the rear doors without changing your routine.
Out of curiosity, what year, make, and model of van are you planning to install it on?
