I have a 2004 Jeep Grand Cherokee that I would like to tow with the Falcon 2 hitch that came with my motor home. Which base plates do I need? Are the 1425-1 roadmaster the correct ones?

As long as your Grand Cherokee isn't the SRT-8 model, the 1425-1 base plate kit would work just fine for you.
In order to safely flat tow, you'd also need the following:
Safety Cables like # RM-643
Wiring Harness so Taillights of Towed Vehicle Operate with RV part # RM-154
Supplemental Braking System like the Roadmaster Even Brake # RM-9400
