The Curt Trailer Hitch Receiver # C14357 is confirmed to fit all models of the 2004 Ford F-150 unless you have a factory hitch installed. What likely happened is that the hitch flexed during shipping. This isn't uncommon since the brackets are designed to allow the hitch to flex with the vehicle after installation. The good news is you can bend it back.
Start by inspecting the hitch to make sure you don't see any cracks or chipped paint. These are signs that the hitch may have flexed enough to be considered unsafe and should be replaced. Assuming it is still in good shape, you can use a table vice or a number of other methods to bend it back into place. I have attached a helpful article below which shows all of the different methods you can use to get it back into position.
