What make/model/weight capacity 2 5/16 hitch ball does the Equal-i-zer EQ64FR, model 90-00-1600 hitch come with? Thanks you.

The hitch ball for the 16K Equal-i-zer Weight Distribution System w/ 4-Point Sway Control # EQ64FR can actually be found as part # EQ54FR and is rated for 16,000 lbs just like the weight distribution system.
This same hitch ball is likely used for some of the lower-rated systems from Equal-i-zer as well. Basically if the system comes with a 2-5/16" hitch ball and isn't rated for more than 16K it likely has this hitch ball.
