In order to get replacement hardware for your hitch you need the Replacement Hardware Kit for Trailer Hitches # RHK. This will require the hitch item number as well as the manufacturer in order to get the correct parts.
The part number you provided does not correspond with any hitch I have been able to find. However, we do have one hitch for the 2009 Toyota RAV4 that matches the capacities you listed, which is the Curt # C13149. You will still need to confirm if this is the right hitch before ordering replacement hardware. The part number from Curt would also be listed as 13149.
