Given your 3 inch diameter axle beam, you have a 5200, 6000 or 7000 lb rated axle, which is compatible with a 12x2 inch brake assembly.
A 12x2 brake assembly would use a standard 5 bolt brake mounting flange. For a 3 inch diameter axle beam, I'd recommend using # 4-44-1 for 3 inch axles.
You'll want to have a professional welder who's experienced in trailer work install these. If they aren't installed so they're perfectly straight and concentric, you'll have no end of trouble.
