Need help picking the correct marine trailer jack. Currently there is a welded on piece with a threaded bolt and four holes 3 inches apart forming a square. Can also send a picture to help.

Thank you for providing the picture of your existing mount. This was helpful to idenfication and I have attached it to this post. As you mentioned, the 90 degree snap in holes are 3" apart from center to center. It may be helpful to note that the center bolt is welded in at the manufacturer.
The part that is welded on your trailer is the Fulton # P9012-00 Mounting Bracket. If you no longer have the stepped bushing and locknut you can order the Fulton # F0933325S00 Replacement Hardware Kit. I would clean up that bolt real good with a wire brush. If you ever have to replace the bolt it will require cutting the mount off of the trailer frame, and cutting the bolt off of the mount!
With this bracket you can use the Fulton # FXP15W Swivel Jack with 1500 pounds capacity and 10" lift, or
the Fulton # F141132 swivel jack with 1500 pound capacity and 12" lift.
Besides the lift difference of 2 inches, I think for a boat trailer I would prefer the better Zinc coating on the FXP15W.
