With clearance being the biggest issue for you, there is one option I highly recommend for a 5th wheel hitch that fits your 2019 Ram 2500 with a 6-1/2 foot bed, the Demco Hijacker Autoslide 5th Wheel Trailer Hitch with Slider # DM8550034.

The Hijacker gives you more clearance than any other sliding hitch because of its 14 inches of front-to-back travel; other hitches have between 10 inches and 12 inches of travel. That will give you the most possible clearance for your 2019 Ram, and you aren't sacrificing quality or capacity with the Hijacker, which has an 18,000 lb capacity, and will work in reverse, and you'll never have to get out of your truck to make a turn with the autoslide feature.

The Hijacker also has a unique, single-piece jaw that wraps totally around the king pin, and that connection is reinforced via a 3/4" solid steel bar that locks into place once the jaw is closed. There isn't an option for underbed rails with the Hijacker, but there is one option for a 5th wheel hitch and rails that are below the bed for your 2019 Ram 2500: - B&W Companion Slider Fifth Wheel Hitch # BWRVK3400-5W - Custom Underbed Installation Kit for B&W Companion 5th Wheel Trailer Hitches # BWGNRK1320-5W You mentioned having a B&W Companion for your long bed truck so you already know about the quality of this hitch. It has a hitch height that is adjustable between 17 and 19 inches, and does have 12 inches of front-to-back travel, which is a couple of inches less than the Hijacker.
