2018 Highlander XLE. Does not have trailering package. To add the wiring harness are all Highlanders equipped with plug and play after removing all the plastic pieces by the spare tire tools?

Installing a 4 pin trailer wiring harness is a plug and play proposition on all models of your generation of Highlander.
Since your Highlander doesn't have the trailer package, you'd want to use # 118449 which installs using the two connectors under the cargo floor right at the threshold for the rear hatch. The kit includes the converter assembly, the trailer connector that plugs into the other connector and the fuse that would need to be installed in the fuse box in the engine compartment. Had your Highlander been equipped with the trailer package, the # HM11141825 is what you would use.
