The TorkLift Cannon Hitch Extension # TLE1648 does not feature any kind of rise as the extension gets closer to the 2-1/2" receiver. This particular extender, which is designed specifically for Ford factory 3" hitch receivers, does feature a unique set bolt underneath the shank that tightens up to the receiver at the connection point. This component, along with the lateral support chains that will attach to the hitch, help to drastically reduce any sag that a standard extension of this length would otherwise undergo. This is also why, as opposed to most other similar adapters, this extender maintains the weight capacity of your trailer hitch when used.
I have attached a short video review that you can check out as well.
