It definitely sounds like the internal gears on your existing landing gear system are shot so a complete replacement is your best option. I'm not able to track down what specific model system would have been installed at the factory on your 2004 Carriage 5th Wheel Trailer but I can absolutely help you find a compatible replacement.
The Bulldog Tandem Landing Gear Set # BD500162 for manual operation that you were looking at will likely work great. It features standard weld stop/bracket dimensions of 15-1/2 inches from inside to inside and 17-11/2 inches from outside to outside (see attached image).
What you can do is measure your existing brackets to make sure these will bolt right up to them. If you happen to have different spacing just let me know and I can see if we have a different system that will match up. Otherwise you can simply cut off your current brackets and weld on the new ones that will come included with the landing gear.
I have included a short video demonstration and the written installation/operation manuals that you can look through.
