I bought a used hitch and need the template peice cm-60604-t to drill the hole where can I get that part

The gooseneck hitch you have is the Curt 600 Series part # C60604 which is just the rail kit of the # C607-604 that you referenced. I attached a link to the install instructions and an install video for the hitch for you to check out as well.
If your truck was a short bed the center of the gooseneck hole would need to be 44 inches from the rear truck bed rail. If instead you had a long bed the dimension would be 48-7/8 inches instead.
