I do have a 5th wheel installation kit that is a fit for the 2012 Chevy Silverado 2500HD, but the CURT kit you have referenced has not been verified as a fit. You can use the Reese Custom Installation Kit, # RP50066-58. I have included a link to the installation details and a link to a video showing a typical installation.
