The Yakima BlockHead # Y01117 works with some bikes with disc brakes but not all. Since I can't guarantee that it will work with your specific disc brake bike I recommend looking at some other options. The similar Hollywood racks carrier, # HRT970, shows that it is compatible with both disc brake bikes and non-disc brake bikes so it is one option. I have linked a video review for you.
