I do have a solution for you, but unfortunately we do not have any Multipurpose Ball Mounts like the Curt # D210 that feature a 4" drop.
Instead what you can use is a Gen-Y adjustable ball mount like part # 325-GH-523 which has a 5" drop /rise and a 16K weight rating. If you are within 1" of the rise/drop needed for your towing application then you can use the ball mount. This means that you can use the included ball mount in the lowest position thich frees up the top receiver opening for an accessory, provided that your coupler allows an accessory to fit.
Please note that you will be limited to the weight capacity of your tow vehicle. Even though this Gen-Y receiver is rated or 16K doesn't mean that your vehicle will get a boost up to 16K for weight capacity.
I have attached a demonstration video for you to reference.
