You likely have 5,200 pound de-rated axles. You can check the bearing numbers stamped into the sides of the bearings in one of the hubs on the trailer now to help determine what will fit. Most likely they use inner bearing 25580 and outer bearing 15123.
For a disc brake kit that uses these bearings I recommend # K2HR526DKG. This is a kit for one axle so if you need brakes on both axles you would need 2 kits.
For bearings you would need 4 of inner bearing # 25580, 4 of outer bearing # 15123, and 4 seals. If your spindles take a 2.125 inch inner diameter seal use # GS-2125DL. If they take seals with 2.25 inch inner diameter use # GS-2250DL.
If your trailer hubs have different bearing numbers let me know. The above disc brake kit has a 6 on 5-1/2 bolt pattern. If your wheels take something different let me know.
I assume that your trailer currently has hydraulic drum brakes. If so you can reuse the same brake lines but not the same actuating coupler. There may be a replacement, disc brake compatible master cylinder that you can install depending on the make and model of the coupler currently on the trailer. But if there isn't then you would also need to replace the coupler with one that is disc brake compatible such as # T4750620.
If the trailer has electric brakes now then you would also need a brake line kit and a coupler like the one mentioned above. For brake lines you can use kit # 20TA-BLKIT. I have also included a link to our help article on adding disc brakes to a trailer that covers everything.
