What ball mount is recommended for this 2017 Pacifica hitch? If the height of the hitch is 12 inches and the standard height of a trailer is 17, should I be looking for a 5” rise mount?

Your assumption is correct. If the distance from the ground to the top of the receiver opening on trailer hitch # 36798 is 12 inches, and the distance from the ground to the bottom of a level trailer coupler is 17 inches, you would need a ball mount with a 5 inch rise.
Ball mount # 36065 is a good option. It takes a ball with a 3/4 inch diameter shank. For a compatible 1-7/8 inch ball you can use # 19256. For a 2 inch ball use # 19258.
For trailer wiring if your 2017 Chrysler Pacifica is a Limited, Touring L, or Touring L Plus use # 118712. If you have an LX or regular Touring model use # 118723. And if you have a Touring Plus (not to be confused with the Touring L Plus) use # C59496.
