Where are the holes for the two screws needed to mount cup holder?

The Camco Adjustable Drink Holder, Item # CAM44043, mounts with two screws which are included. For convenience and ease of use, the two mounting holes are down the center of the back and available in three locations: left, center, right. Choose which location works best for you and then screw down for installation.

It also includes double-sided tape for mounting if you do not want to use screws.

I have attached a video that reviews the Camco holder and clearly shows the different mounting locations and options.
