What rail kit do i need for the patriot 18k for my new to me topkick? Currently have a companion hitch in my pickup. But the topkick has a solid recessed mounted gooseneck so i will need to go to a rail kit and patriot correct?

We don't have a custom rail kit that fit your C4500, but at this time we do not have anything that would work aside from recommending you use a universal kit part # RP30035 to come up with a way to install the Patriot.
I looked up pictures of your Topkick and it looks like they used a similar universal kit to the # RP30035 to install rails and then use a hitch like the Patriot part # BWRVK3255.
