It sounds like you're looking for the ceiling garnish for a standard roof vent like # VA0445-34.
I've linked our selection for you. Just choose the color and the measurement of the leg you need. The leg is essentially how tall the garnish is. If you can't find one the exact size you need you can purchase a larger one and cut it down to size.
If you're looking for the complete vent assembly our most popular is the Ventline # V2092SP-28. It's a manual lift vent.
If you need a vent with a fan instead use # V2094SP-28.
