I spoke with my contact at Curt and the Double Lock, Flip and Store Underbed Gooseneck Hitch with Installation Kit, # C60712, will fit your 2004 Chevrolet Silverado LS 1500 Crew Cab with the 5 ft 8 in short box.
Since this kit has been tested on a 2004 with a standard box and the frame holes are the same for the short box, we do know that this kit will work however we do not have instructions/measurements for cutting the ball hole on the short box. In this case you would install the underbed hitch and then take an underbed measurement from all 4 corners of the box/bed to the center of the hitch ball. Next you will duplicate these measurements in the bed of the truck to determine where to drill the hole for the ball. I highly recommend that once you have the spot marked, drill a very small hole to make sure that you are in the right spot. Once you know that everything is good, it would now be safe to drill the 4 inch hole.
