I was given a Thule Mountaineer cargo box. I do not know if it is an older 669 or a 669ES. How do I determine that?

You would need to look at the hardware. The 669ES will have easy snap hardware which will look like the part # 43216 that you referenced.
The 669 would have installation hardware that would look like part # TH04997 instead.
