What is mod wheel and how is it diff than reg wheel.

Mod wheels such as the one in tire and wheel combo # LHACK121 have the distinctive round holes. A "regular" trailer wheel on the other hand has more triangle shaped holes and is referred to as a "spoke" wheel. Take a look at # LHAW124 as an example. So basically you are looking at an appearance difference. I have included links to video reviews of both for you.

Michael, Is the hole measurement distance the same on the 5 Lug modular rim and the 5 Lug spoke rim?

@Charles Are you speaking of the holes for the wheel studs or the round holes in the mod wheel and the triangular spoke holes of the spoked wheel? In a 15 inch wheel, the mod wheels typically have 10 holes while the spoked wheels have 8.
