2019 Ford F450 Lariat, 8foot bed, with factory underbed prep for 5th wheel. Does the Bed mat have pre-perforated locations marked on the underside to cut out for 5th wheel pins?

No, I am sorry it does not Dave. None of our mats like the DeeZee Custom-Fit Truck Bed Mat item # DZ35GJ come with the puck pattern shown on the mat.
