Hello Shawn,
For your 2004 e-450 Ambulance the best hitch for you is the Adjustable Width Trailer Hitch Receiver for RVs, 18" to 51" Wide # C13702, I know you said you were looking for a Class IV but the best option we have is a Class 3. The hitch itself can still handle a 5,000 pound towing capacity, when using a weight distribution it bumps it up to 6,000. I went out to the warehouse to double check that you will have at least 3 1/2" from the frame to the top of the receiver. The angle of my picture is not the greatest, from the top where the frame will be to the bottom of the tube is 4 1/2", then the receiver is going to sit right below that so you will even have an inch to spare.
What are you looking to tow with the hitch that you are looking for a Class IV?

Hey Shawn, how did things go with the adjustable width hitch on your E-450 ambulance? Did it give you the clearance and towing strength you needed for your setup? Your update could make it easier for others working with similar frame measurements.
