Is the 6004 the correct adapter plates required for installing the 21k Demco Autoslide hitch to a 2015 Chev High Country factory installed OE 5th wheel prepped truck?

Hi Rick; there wasn't an actual OEM prep package for your 2015 Silverado 2500 so I'm guessing you have the Underbed Rail and Installation Kit for Demco UMS 5th Wheel and Gooseneck Trailer Hitches # DM8551010. Take a look at the dimensions below and see if that matches what is on your truck. Assuming it does, the Replacement Side Plates # DM6004 are what you need. If you're looking for the full hitch with the side plates we also have the # DM8550022.
