Question:
Will I have to drill a fourth hole on a 2018 Silverado 1500?

Yes, you will. The longer length of the K-Source Custom Extendable Towing Mirrors item # KS62147-48G over the factory mirrors requires you to drill an extra hole for an additional support. These mirrors are confirmed to fit your 2018 Chevy Silverado 1500.
I have linked an installation video below that we did on a 2015 Silverado along with a diagram outlining the drilling steps. If you skip ahead to the 3:50 mark you'll see the process of getting this hole drilled. The installation will be the same on your 2018 Silverado.
